Idaho weighs 2026 Medicaid reductions

Idaho Governor Brad Little has proposed significant Medicaid budget reductions as part of the state’s 2026 fiscal plan, framing the cuts as a way to close a projected budget gap while continuing Idaho’s voter-approved Medicaid expansion. The proposal outlines multiple reductions that, taken together, could reshape Medicaid spending levels in the next fiscal year. State lawmakers are now reviewing what additional savings may be needed and which services could be affected as the legislative session continues.

Proposed cuts in the 2026 fiscal plan

Governor Little’s plan includes making a previously implemented provider reimbursement reduction permanent. Specifically, a 4% provider reimbursement cut that has already been put in place would continue on an ongoing basis under the proposal. In addition, the fiscal plan calls for further reductions to Medicaid services. The proposal would cut an additional $22 million from Medicaid services. Combined with other planned changes, total Medicaid reductions could reach approximately $45 million.

Lawmakers asked to identify more savings

Beyond the reductions already approved and outlined in the plan, state lawmakers have been instructed to identify additional savings. Health and Welfare committees are reviewing which Medicaid services could be reduced and how those changes may impact Idaho residents. This review process is expected to shape the next phase of the debate, as legislators consider the scope of reductions and the practical effects on people who rely on Medicaid coverage and related services across the state.

Concerns about downstream costs and federal funds

Advocates and some legislators have raised concerns about the longer-term consequences of reducing Medicaid funding. They warn that cuts could increase long-term costs, particularly for hospitals, emergency services, county health systems, and families. There is also concern that federal matching funds could be reduced if state spending declines, adding another layer to the fiscal and policy considerations in front of lawmakers.

As the legislative session continues, debates are expected over how to balance fiscal responsibility with maintaining access to critical healthcare services for vulnerable populations across Idaho.
