North Dakota seeks input on IDD waiver

North Dakota Health and Human Services has opened a public comment period on a proposed amendment to the state’s 1915(c) Traditional IDD Home and Community Based Services waiver. The comment window runs from January 26 through February 25, 2026, and applies to updates that would affect how services are delivered and administered under the waiver. If approved by the Centers for Medicare & Medicaid Services, the proposed changes are expected to take effect on June 1, 2026.

Public comment window

North Dakota Health and Human Services is accepting written comments on the proposed amendment through February 25, 2026. Comments can be submitted by email, mail, or phone to the Developmental Disabilities Section of North Dakota Health and Human Services. The state’s public comment period is tied specifically to a proposed amendment to its 1915(c) Traditional IID/DD Home and Community-Based Services waiver, which is a key Medicaid waiver authority used to support people with intellectual and developmental disabilities in home and community settings.

Proposed Host Homes service

A central element of the amendment is a proposed Host Homes service. North Dakota Health and Human Services describes Host Homes as a service designed to provide private home placements that support individuals during times of need. The proposed service is also intended to help families manage crises. In addition, it would prioritize reunification with family whenever possible.

Other waiver updates and timeline

Alongside the Host Homes proposal, the amendment includes revisions to provider requirements and administrative language. North Dakota Health and Human Services said these revisions are intended to better align waiver language with current practices and federal guidance. The amendment also includes terminology updates, which are intended to clarify staff roles and program operations.
